Parents or legal guardians may apply online or at their local Head Start or Early Head Start locations. Our program prioritizes families facing significant challenges to ensure services reach those with the greatest need. Special consideration is given to children in foster care, children with documented disabilities, and children in unstable housing situations.
Ways to Apply

Print & Submit – Download, print, and complete both sides/pages of the Head Start Program Application (English Version) (Spanish Version) (click link). Incomplete applications cannot be processed. Only expectant mothers, parents, or legal guardians may sign the application(s).
Required Documents
Proof of Income (Submit one of the following:)

Last 12 months of consecutive pay stubs

Most recent 1040 Income Tax return (front page only)

W-2 form(s)

Employer Verification Form (available upon request)

SNAP/TANF/SSI verification (Notice of Action letter)
Note: TANF benefits (food stamps) are not considered income but should be marked on the application if received.
Child’s Birth Certificate (copy required for each child enrolled)

If born in Nebraska, a copy of the Immunization Record may be used temporarily until the birth certificate is obtained.
Up-to-Date Immunization Records (copy required for each child enrolled)
Submitting your application(s)
Once completed, bring the application and required documents to your nearest Head Start or Early Head Start location or the Agency Main Office below. Be prepared to discuss your application and eligibility with a staff member.

What happens next?
We must be able to reach you in order to enroll your child(ren). If you move or change your phone number after completing an application, it is your responsibility to notify the Head Start Program.
You will be contacted, either by phone or by mail, regarding the status of your application(s).
